MODERN MIDLIFE MEDICINE

Your Care Journey with Lumani Wellness

Every woman’s story is unique — and so is the way her body moves through the perimenopausal and menopausal transition. My goal is to help you understand why you feel the way you do, and to give you the clarity and tools to feel vibrant again.

Initial Consultation

Your first visit is a 60-min. deep dive into your symptoms, health and family history, and goals. We’ll review labs, discuss what’s happening physiologically, and outline a personalized plan for your peri- and menopause journey that may include:

  • Hormone or Menopausal Hormone Therapy (MHT) I prescribe bioidentical estrogen, progesterone and testosterone for systemic therapy as well as vaginal hormones to help with vaginal issues that are common in menopause. I prescribe both FDA approved and compounded formulas.

  • Peptide protocols

  • Targeted supplements

  • Lifestyle guidance around nutrition, movement, sleep, and stress

This session is both educational and collaborative — so you understand the why behind your plan.

Follow-Up Visits

Healing and hormone balance are a process, not a prescription.
In the first six months, most women see me 3–5 times as we fine-tune your customized treatment plan:

  • 2–4 weeks after your initial visit: 30–60 minutes — early review and adjustments

  • 8 weeks: 30 minutes

  • 12–16 weeks: 30 minutes

  • Then every 3 months, and after your first year, typically every 6 months

This rhythm allows for steady progress and meaningful connection, with your care continually evolving alongside you.

For those who desire more time or touchpoints between visits, extended or additional follow-ups can be arranged with ease.

Practice Partnership

Every patient in my practice participates in a $75 monthly Practice Partnership, which sustains the high-touch, responsive nature of care. This includes:

  • Secure messaging with a response within 24–72 hours

  • Prescription refills and care coordination

  • Ongoing access to me between visits

  • The ability to maintain a small, attentive practice without rushed, transactional appointments

Because I work closely and intentionally with each woman, I limit the number of new patients I accept — ensuring each patient receives the time and attention she deserves.

Dr. Sarah de la Torre

MD, FACOG, DipABLM

Hi, I’m Dr. Sarah de la Torre — a double board-certified physician with more than 25 years of experience supporting women’s health. After delivering over 4,000 babies and caring for women across every stage of life, my work now centers on helping women in perimenopause and menopause feel strong, vibrant, and confident in their bodies.

Through years of listening to women, I’ve seen firsthand that midlife is both deeply transformative and too often overlooked. That’s why I’ve brought together my training in OBGYN, Lifestyle Medicine, Functional Medicine, and Menopause Care to create an approach grounded in evidence, compassion, and real connection.

My goal is simple: to help you understand what’s happening in your body — and feel empowered to take charge of your health and well-being.

Commonly Asked Questions

A few of the most common questions about how I practice, what to expect, and how to get started.

  • My practice is built around time, thoughtfulness, and truly individualized care — the kind that’s rarely possible in conventional insurance-based models.
Traditional insurance structures reward volume and speed, often allowing only a few minutes per visit. I’ve chosen a different path: one that prioritizes connection, education, and a deep understanding of what’s happening in your body.

By working outside of insurance, I’m able to spend the time it takes to listen fully, create an integrated plan, and stay responsive as your care evolves.
This allows for the high-touch, holistic, and personalized care my patients value — without the rushed pace or limitations of traditional systems.

  • I am licensed in ID, AZ, WA, CA, IL, OH, CO, MT and MA is pending.

If you live outside these states, you can still schedule an educational consultation. In this visit, I’m not acting as your medical provider — I can’t prescribe medications or order labs — but I can listen to your story, review your health goals, and outline a thoughtful plan you can take to your local clinician.

It’s a great option for women who want expert insight and direction, even if we can’t formally establish a patient–provider relationship.

  • I typically order labs through LabCorp or Quest, which can be billed through your insurance. I include appropriate diagnosis codes, so insurance is more likely to cover the cost.

In some cases, it’s more affordable to order labs direct-to-consumer, for example, through Fullscript, where I can create customized lab orders that you pay for directly.

If you’ve had recent testing done elsewhere, you can upload your existing lab results to your secure chart once you’ve completed your intake paperwork. I’ll review them carefully, so we avoid unnecessary repeat testing.

Most labs are ordered after our first visit, once I have a clear sense of what’s needed. I also like to follow up on hormonal labs after you’ve been on treatment for a period of time to track your response.

While symptoms guide the majority of our decisions, labs often help us put the puzzle pieces together and fine-tune your care plan.

  • I offer telemedicine exclusively for the convenience and flexibility of my patients. You don’t have to drive, find parking, or sit in a waiting room, you can connect with me from wherever you are, whether that’s home, work, or while traveling.

Virtual visits allow us to stay consistent and connected, even when life is busy. This approach makes high-quality, personalized care more accessible and sustainable, without compromising depth or connection.

All appointments are conducted through a secure, HIPAA-compliant platform, ensuring your privacy and comfort at every step.

  • Every new patient begins with a comprehensive initial consultation — a deep, personalized conversation about your health.

We’ll review your medical and family history, current symptoms, and overall wellness profile to get a complete picture of what’s happening in your body. I’ll spend time educating you about perimenopause, menopause, or other hormone-related concerns, so you fully understand what’s driving your symptoms and what your options are.

Together, we’ll create a personalized treatment plan, discuss the risks, benefits, and evidence behind each option, and decide what feels right for you. I’ll prescribe what we agree upon, and we’ll schedule your first follow-up.

Typically, we meet again 2–4 weeks later to review how you’re doing, make any adjustments, and consider lab testing if needed. From there, follow-ups are usually every 1-3 months and then  depending on how you’re feeling, your lab results, and your treatment goals, every 3-6 months.

  • Yes, I offer evidence-based menopausal hormone therapy (MHT) when appropriate, using a holistic, individualized approach. We’ll discuss whether hormones are right for you, the safest and most effective options, and how to combine them with lifestyle and supplement strategies for optimal results.  I prescribe both FDA-approved and compounded bioidentical hormone, estrogen, progesterone and testosterone as well as vaginal hormone formulations, depending on your needs and preferences.

Insurance will typically cover FDA-approved prescriptions, but compounded formulations are often out-of-pocket. I take all of this into account and discuss the options in detail with you, so you can make an informed decision about what feels best for your body, goals, and budget.

My approach combines evidence-based hormone therapy with holistic strategies, including nutrition, supplements, peptides, and lifestyle guidance, to support your whole-body wellness through perimenopause and beyond.

  • As part of your Patient Partnership ($75/month), refills are included in your ongoing care. I generally provide three-month refills at a time to help prevent any interruption in your therapy.

All communication takes place through the secure, HIPAA-compliant chat portal. This keeps messages organized, private, and easy to track. I don’t use text or personal email for medical communication, as those can easily be lost or delay your care.

I respond to messages within 24–72 business hours, and refill requests are processed within 72 hours.

If you need an urgent refill (within 24hours), a $25 expedited refill fee applies.

  • I focus on perimenopause, menopause, hormonal health, sexual wellness, urinary and vaginal health, and the broader metabolic changes that accompany midlife.

Common related concerns include low energy, sleep changes, libido shifts, mood fluctuations, and body composition changes — areas in which I have deep experience and a nuanced approach.

I also have extensive experience with PCOS, fertility, and menstrual concerns such as heavy or painful bleeding and PMS, helping women of all ages better understand and manage their hormonal health.

  • Group educational consultations create a supportive space to learn, connect, and feel empowered in your health journey. Dr. de la Torre shares evidence-based insights and practical guidance on topics like menopause, fertility, and wellness, helping participants gain knowledge, confidence, and community — all without individual medical advice.

  • Each session includes education, open discussion, and time for questions. Dr. de la Torre leads the group through key women’s health topics, offering practical tools and insights you can apply right away. Sessions typically last about 2 hours with a minimum of 5 participants, at a rate of $50 per hour per person. You’ll leave feeling informed, supported, and empowered to take charge of your health.
